YOUR MEMBERSHIP CARD

1. Please take a moment to read the benefit descriptions included in your membership packet.
2. Keep your membership card with you at all times so it will be easily accessible to present at provider locations. You must present your membership card at the time of service to receive a discount.
3. All you legal dependents can use your membership card, or you may order additional cards for legal dependents with their names, for a small fee.
4. To locate participating providers I your area, visit www.locateproviders.com, or call the appropriate number on the back of your card.
5. When you present your card to a participating provider, you will receive a discount at the time of purchase or service.
6. If you have any questions on how to use your membership, call our Member Services department at the toll-free number listed on the back of your card for assistance.
7. Please keep in mind that your membership is NOT insurance – it is simple and easy to use. There are no health restrictions or paperwork.
8. You can use your card immediately and as many times as needed.
